Are you a new or existing NDIS participant looking to access the quality services offered by Hands to Hearts?  Here’s a simple guide on how to nominate us as your preferred provider through the NDIS portal (PACE).

Step-by-Step Guide

1. Log in to your NDIS Portal (PACE):  Access your PACE portal and navigate to the section where you manage your service providers.
   
2. Search for Vana Care: Use the search function to find Hands to Hearts Pty Ltd. Our details are:
   • NDIS Provider Name: Hands to Hearts Pty Ltd
   • NDIS Provider Number: 4050121929
     
3. Nominate as Participant Endorsed Provider (PEP): Select the option to nominate Hands to Hearts Pty Ltd as a Participant Endorsed Provider (PEP). This allows us to provide services and support under your NDIS plan.
   
4. Complete the Participant Intake Form: Download the Hands to Hearts Participant Intake from here and complete it.
   
5. Submit Form and Plan Details: Email the completed Intake Form, along with a screenshot or copy of the relevant sections of your NDIS plan outlining your approved supports, to hello@handstohearts.com.au. We will then send you a service agreement to be signed.
